Preventing coronary occlusion with a retrievable aortic valve.

چکیده
Coronary occlusion is one of the potential complications during transcatheter aortic valve implantation. Second-generation fully retrievable and repositionable devices can theoretically prevent this complication. Figure 1 (A) Delivery system containing the unexpanded Lotus valve through the aortic valve; (B) the Lotus valve partially expanded. A 81-year-old woman with symptomatic aortic stenosis (valve area 0.52 cm 2 ; peak and mean gradients 118 and 70 mmHg, respectively; normal ejection fraction) was referred for transcatheter aortic valve implantation. Recent studies demonstrated an association between aortic valve sclerosis and obstructive coronary artery disease. The purpose of this study was to evaluatethe correlation betweenaortic valve sclerosis andobstructive coronary artery disease and the extent of coronary artery disease in patients ho...
متن کاملBicuspid Aortic Valve: An Unusual Cause of Aneurysm of Left Coronary Sinus of Valsalva
Bicuspid aortic valve is traditionally considered an innocuous congenital anomaly. Due to a better and widespread availability of non-invasive imaging techniques, it has come to the fore that 30% of these cases develop complications, viz., valve abnormality (aortic regurgitation and stenosis), and aneurysm of aortic root and ascending aorta. Sinus of Valsalva aneurysm is an uncommon complicatio...
متن کاملHigh-risk Coronary Artery Bypass Grafting and Mitral Valve Replacement in a HIV Positive Patient
Certain subsets of high-risk mitral valve patients are not suitable candidates for transcatheter therapies. The objective of this report is to present a young patient with combined mitral valve and coronary artery disease to illustrate these challenges. In this report, we present a 47-year-old man with longstanding HIV infection who was referred with severe mitral regurgitation (MR) and profound...

T ranscatheter aortic valve replacement (TAVR) is a well-established therapy that has revolutionized the management of patients with severe aortic stenosis who are deemed suboptimal candidates for surgery.1-3 Despite increasing experience with and understanding of the procedure and patient selection, TAVR is still associated with unexpected complications, such as paravalvular aortic regurgitati...
متن کاملAcute Right Coronary Ostial Stenosis during Aortic Valve Replacement
We report a rare case of acute right coronary artery stenosis developing in a patient undergoing aortic valve replacement. We present a case report with a brief overview of the literature relating to coronary artery occlusion associated with cardiac valve surgery - the theories and treatments are discussed. A 85 year-old female was admitted under the care of the cardiothoracic team with signs a...
